Once you have received an email of approval for an appointment, follow these steps:
Review Practice Policies, Fees and Frequently asked questions before your first visit. (if not done already)
Create a patient account through Patient Portal or an email invitation will be sent to create one.
You will be contacted by us for a first appointment once we have received your prescreening form.
Full payment is required at the time of making an appointment. You can pay by using credit card/debit card/HSA card. With Cigna insurance, you are required to pay the copay at the time of scheduling the appointment. Please check your benefits.
Sign any documents sent before the appointment.
Existing patients can pay through ivypay. You will receive a text from ivypay to pay your visit.
